As an alternative, we can put a hold on your membership for a reduced monthly fee. Or, you may be eligible for a different type of membership and/or level of access. If there is anything we can do before you request your cancellation, please contact us by clicking here instead.
Cancellation Terms:
Member may resign from Active Acquisition Partners, LLC, Active Wellness, LLC and Providence (dba Active Wellness Center, collectively, the “Active Wellness Center Parties”), by giving advance written notice to the Active Wellness Center Parties, subject to the following terms and conditions. Resignation notices received by Active Wellness Center Parties between the first calendar day of the month and the 25th calendar day of the month will become effective 30 days after the date of receipt. The billing for the final calendar month will be prorated to exclude any days falling outside said 30 day period. Resignation notices received by Active Wellness Center Parties between the 26th calendar day of the month and the last calendar day of the month will become effective on the last calendar day of the following month. Dues will continue to accrue until the resignation is effective.
Important Notice: Additional members that are paid by a primary member will continue as active members unless a separate resignation form is completed or alternate electronic funds transfer information has been provided.
